PATIENT PORTAL STEP-BY-STEP TUTORIAL
You are a valued patient here at Aguila Medical Group. With the advancement of technology and convenience it offers, we offer TeleVisit or TeleHealth Virtual Consultation where you will be able to talk to your provider at the comfort of your home. Our EHR (Electronic Health Records) System, AdvancedMD, is secured, HIPAA-compliant and has been widely used by more than 40,000 practitioners across 13,000 practices.
Upon confirming your appointment with us, we will send you a link within 24 hours. Just follow the tutorial below so you can access your Patient Portal. If you prefer receiving the consent forms via e-mail, just send us a text message or call our office. For those with confirmed In-Person Visits and has not yet completed the forms , please arrive at least 30 minutes earlier than your scheduled appointment, so we can assist you. We have also uploaded the form, just click DOWNLOAD FORM (Password is: amg). You may send your completed form at: aguilamedicalgroup@gmail.com and we will send you back a message using our secured email. Thank you very much!
1. Go to your inbox and check your email. Open the PATIENT FORMS NOTIFICATION message sent by AGUILA MEDICAL GROUP.
2. Click View Forms.
3. Click REGISTER.
4. Type your First Name, Last Name, Email and tick the box after reading the terms and conditions. Then, click Register.
5. Check your email again, to confirm your account. Just go to your inbox, and look for a new message from Aguila Medical Group. Open this message, PATIENT PORTAL ACCOUNT NOTIFICATION.
6. This is how the message will appear. Click, CREATE YOUR ACCOUNT.
7. Create your Password, confirm your password and type your date of birth in this format: Month/Date/Year (02/14/1988). Email and tick the box after reading the terms and conditions. After that, click Register.
8. CONGRATS! You will now be asked to log-in with your e-mail and password.
This will be the dashboard of your Patient Portal. You will be able to:
- Request an appointment
- Submit check-in forms before your appointment
- Send messages to and receive messages from your office
- View prescriptions and request prescription refills
- View account balance
- View medical history
- View patient education materials plus more!
